School Exams: Illinois requires all students entering PreK, Kindergerten, 6th grade and 9th grade, and if a child will attend a new public/private school to have a well child visit with completion of the required school form. We have this form in our office. We recommend that school exams be done in the spring to early summer prior to entering school. Some schools require completion of forms by early/mid spring for registration. Please schedule your school physical appointments now by calling 708-484-0042. If your child is on medication that needs to be given during the school day, please bring the appropriate form to your well visit. If your child sees an allergist for asthma/food allergies, the allergist should complete these forms for you.
Sports Exams: Illinois requires all high school athletes to have a pre-participation physical in order to confirm that your teen is medically cleared to participate in their sport(s.) This form is only valid for 1 year from the date of the exam with your physician. Please print the form (hyperlink below) complete all parent sections (bottom of page 1, pages 2-4 prior to your visit with your teen AND bring this form with you. Your physician will complete the top of the 1st page, last page and give you the completed form. https://www.ihsa.org/documents. If your child is younger is not yet in high school, some sports have their own forms and others will still accept the IHSA form. Please check with your sports team and bring any necessary forms to your well visit.
Camp Physicals: Please complete all of the parent information sections before bringing your child for a camp physical. Please bring these forms to your well child visit.
Of note, if school/sports/camp/medications forms aren't available at the time of the well child visit, there is a form charge of $20/form for each form that needs to be completed.
