Your Medical Home

We strongly believe that each patient should have a primary care doctor that cares for all of your ongoing medical problems. Primary Care Associates will be your Medical Home. We will facilitate partnerships between individual patients, their personal physicians, and when appropriate, the patient’s family.

Our Role in Your Healthcare

Our Healthcare Team will be dedicated to you. We will provide:

  • Personalized Care because our physicians and office team know you and your health history
  • A Safe Environment in which to discuss your private medical concerns
  • Explain services offered at Primary Care Associates
  • Guide you through the complex healthcare systems
  • Coordinate your care within our office, among specialists, the hospital, and with other healthcare services

 

Your Role as a PCA Patient

We encourage Active Participation in your healthcare decisions.

  • Discuss your concerns at the time of your visit
  • Provide us with a list of all medications (prescription and nonprescription) in order to help prevent medication interactions
  • Call us before you go to the Emergency Room or Urgent Care Center
  • If you seek care at another facility, please make sure that they know who your doctor is at PCA and ask to have any records or immunizations sent to our office via fax at 708-749-5489
  • Be informed about your healthcare decisions
  • Be compliant with follow-up visits, recommend lab tests or procedures, and specialist visits
