New Patients Pediatrics

We are excited that you have chosen Primary Care Associates for your child's medical care. Your comfort and convenience are our priority, and we strive to make every visit to our office a positive experience. To help you get acquainted with our office and first visit procedures, we have included helpful information on this page.

What to bring to your first visit?

We work hard to ensure that you and your family receive prompt medical care in a comfortable office setting. To help expedite the first appointment, we ask that you please bring your insurance information and medical records that haven't been previously sent. Please come 10 minutes before your first visit in order to complete all necessary forms.

Pre-Visit Preparation:

Please send the following medical records before your first visit to our office. These records can be faxed to our office at 708-749-5489. You may also bring a copy of these records to our office in advance of the visit. Please label the records with the name of the physician with whom your child has an appointment. We will try to have pertinent information loaded into our Electronic Healthcare Record in advance of your visit. We do not need the entire medical record. We request the information noted below. 

  • Immunization Records
  • Growth Charts
  • Most recent lab results (lead level, blood counts, or blood tests)
  • Most recent Consultation and Surgical Notes from a specialist, if applicable
  • Newborn hospital records for newborns
  • Most recent well child exam from your prior physician

Vaccination Policy

We are happy to discuss your concerns about vaccines at your child's next visit. All children in our practice have received recommended immunizations. If your choice is not to immunize your child, please choose a different medical provider for your pediatric and adolescent care.

What to Expect

A pleasant, comfortable first visit builds trust and helps put the child at ease during future visits. We want your child to enjoy getting to know our doctors and staff, so we work hard to establish this bond during every appointment to our office. During your initial appointment, we will explain everything in detail and answer any questions you may have.

We look forward to meeting you and your child and providing the quality, comprehensive medical care you expect and deserve.

Directions

Primary Care Associates is located at 6840 Windsor Avenue at street level. Parking is available on Windsor Avenue, Kenilworth Avenue, or at a Public Parking Garage on Grove Avenue, two blocks from the office. There is a disabled parking space directly infront of our office.

Local Landmarks - 1/2 block west of MacNeal Hospital; Karasek Park is on the west side of Primary Care Associates

Public Transportation - The Berwyn Stop for the Burlington Northern Train line is 1 block east of the office. The 311 Oak Park Bus stops directly in front of MacNeal Hospital.
