Our main phone number is 708-484-0042. When our office is closed, and you have an issue that can not wait until the office reopens, please call 708-484-1027 and you will be connected to the physician on call.

You may also contact us by secure messaging; please sign into our patient portal. You may contact your physician on our
secure web portal for non-emergent issues, our phone staff to schedule an appointment, our referral coordinator for any pending authorizations, and our billing office with any financial concerns. You may also pay your bill on line. This will ensure that all of your information will be saved in a manner that will guarantee your medical and financial information is safe, secure and protected.

Once you get to the portal you may contact your physician by clicking on their name as well as the following areas of Primary Care Associates.
- Business Office – will connect you to our billing office.
- Scheduling Desk - will connect you to our scheduling desk for NON SAME DAY cancellations or for future appointments. Please indicate the physician, day, date and general time that you would like to be seen along with a reason for the visit.
- Referral Desk- will connect you to our insurance authorization coordinator
- Medical Records – will connect you to our medical records personnel.
- Office Administrator – will connect you to a member of our management team
